We can mail you the forms or fax them to you, if walking in to sign up is inconvenient for you. You may then return them to us in three ways.

  1. Walk them in: We verify the forms are correctly completed and you can pay your first month's service fee at that time.
  2. Fax them in: You can fax us your registration forms at 382-3762. We require you send us the original copies, with your signature(s), via mail within five days. Along with the original copies, you must send in your first month's payment.
  3. Mail them in.

You also need to mail a check or money order for the first month's service. Please do not send cash via mail.
