Midwest Insurance Consultants, Ltd. has our sign up forms available at the following three offices:

  1. Nevada: 1125 6th Street, 382-3541
  2. Maxwell: 93 Main, 387-8725
  3. Collins: 209 Main, 385-2203

You have three ways to get the forms back to us:

  1. Walk them in: We verify the forms are correctly completed and you can pay your first month's service fee at that time.
  2. Fax them in: You can fax us your registration forms at 382-3762. We require you send us the original copies, with your signature(s), via mail within five days. Along with the original copies, you must send in your first month's payment.
  3. Mail them in.
