The Nevada Chamber of Commerce has our sign-up forms available on the counter when you enter their building. Nevada Chamber of Commerce is located at 1015 6th Street. It is next door to State Bank & Trust.
To get the forms back to us, you have three choices:
Walk them in: We verify the forms are correctly completed and you can pay
your first month's service fee at that time.
Fax them in: You can fax us your registration forms at 382-3762. We require
you send us the original copies, with your signature(s), via mail within
five days. Along with the original copies, you must send in your first month's
payment.
Mail them in.
You can mail the forms to the following address:

You also need to mail a check or money order for the first month's service. Please do not send cash via mail.
