.: Signing Up With Us : Nevada Public Library :.

The Nevada Public Library has our sign-up forms available in the brochure rack when you enter their building. Nevada Public Library is located at 631 K Avenue. It is across from United Methodist Church on the northwest corner of the intersection of K Avenue and 7th Street.

To get the forms back to us, you have three choices:

  1. Walk them in: We verify the forms are correctly completed and you can pay your first month's service fee at that time.
  2. Fax them in: You can fax us your registration forms at 382-3762. We require you send us the original copies, with your signature(s), via mail within five days. Along with the original copies, you must send in your first month's payment.
  3. Mail them in.

You also need to mail a check or money order for the first month's service. Please do not send cash via mail.
